Provider uses a custom form that includes annual review and annual custom data on it. When finished entering the data in the form the annual review data and annual custom data do not appear to be saved in the client records. (The data are not found when looking at the annual review or annual custom tabs). This is true even if CAREWare is exited and then reentered before looking at the client annual review record.
